35 years old male patient with history of schizophernia and bipolar admitted to Kindred hospital present on the tele-health . The patient stated that he hear voices that he is god,last voice heard was yesterday. The patient mood was cool, not marry but has a 16years old son.

Therapeutic intervention

Medication management: Risperidone 0.5 mg bid, depakote 500 mg bid

17 year old female patient with history of MDD and anxiety present in-person to the clinic with mom. The patient was admitted to Lurie children hospital last year. The patient has sucidal taught because of been inside and online classes during pandemic. The patient was in clinic because of a prescription of lexapro that she try to refill, has no allergy, lose interest in things,sad, and has sleep disturbance. Dad has bipolar disorder and mom has anxiety disorder. Patient stated that symptoms are way better and can control it, patient rated depression 1 and anxiety 5.

Medication management: Lexapro 10 mg daily.
