Case Study Scenario:An 80-year-old female widow lives by herself in a small, government apartment building. She has two sons (50 & 60 years old) who are both married, have families and unfortunately live more than an hour away from her house. She has breast cancer which has been managed for the last 20 years. She visits the nearby Federally Qualified Health Center (FQHC) for follow-ups every three months. She has missed a few of her appointments, and lately, she has been losing weight, feeling tired, and depressed, cant sleep well at night, and has no appetite. She made an appointment to visit her doctor at her routine FQHC.

Based on your knowledge and research, what impact does your role have on patient care and the healthcare
system as a whole? (max 250 words):
When private and government institutions join, each investor makes a difference in target patient care and
the whole health system. Offering financial resources for FQHCs safeguards the accessibility of healthcare provision,
even for groups such as older people who cannot afford them. The money we waste directly affects the availability of
preventive care services, disease management, and mental health support, which are crucial in improving patients’
outcomes and quality of life.
Besides, we use our healthcare investing to promote the sustainability and integrative use of our natural
resources. The level of competency of the health care system. Their adequate funding allows healthcare facilities to
maintain staffing levels, upgrade medical devices, and implement developing programs adapted to the needs of the
patients (Matthew Fisher, 2020). Through the financing of programs to ensure health person-centered casing
management, chronic disease management and patient engagement, a healthcare delivery system that is more
collaborative and coordinated will emerge. Therefore, the role of funders as investing entities should be highly
valued in making healthcare services reasonable across different populations, strengthening patient-centered care,
and building the healthcare system (Leider, 2020). With intelligent use of resources and cooperation with healthcare
providers, we can be more efficient in taking care of the patients and be in favor of the longevity of the healthcare
delivery.
What contributions will your role make to the case study and your team? Be as specific as possible. (max 250
words):
In this case, as funding institutions, we can incorporate several essential aspects that could help in patient
care and the multidisciplinary team engaged in her recovery process. Firstly, we shall provide activities to enhance
communication and collaboration between the patient’s primary care physician and specialists supporting the
management of breast cancer. We can achieve the same objective if we support the implementation of electronic
health records systems or telemedicine platforms where there will be little or no barriers to information sharing and
the comprehensive meeting of a patient’s healthcare needs.
We can also consider allocating our finances to installing services that will facilitate easy access to
nutritional counseling, transport assistance, and social programs that match the needs of the elderly living alone and
on their own (MAANI, 2020). Through community-based organizations and partnerships with local governments,
we can create links that will ensure that health service delivery is not restricted within a clinical environment but
addresses the social determinants that can affect a patient’s health.
The financial support of healthcare initiatives gets us involved in making policies that foster
patient-oriented care, including team-based care and shared decision-making (Masefield, 2020). By motivating
healthcare providers to adopt a wide-ranging approach in dealing with the patient’s physical, social, and mental
wellness needs, we will ensure that the interrelated components of the overall health of the patient are addressed
appropriately.
