An example of a deficiency is during survey, it was noted throughout an entire day a residents foley could been seen from the hallway. This is considered a deficiency because a courtesy bag should be covering the foley.
Since I work in a nursing home, I know that in order to fix these problems, a plan of corrections must be submitted to the health department and approved. The plan of corrections will detail how the home plans to change/fix the problems and how they will prevent from this problem happening again.

The nursing home I chose to discuss for my assignment is Hallmark Living Holland in Holland, Michigan. Hallmark Living Holland is a medium facility that is owned by a for-profit corporation with a resident council that also accepts Medicare and Medicaid. This facility is rated horrifically and has a slew of complaints, citations, and penalties (Medicare.gov, n. d.).
Hallmark Living Holland’s most recent health inspection, occurring in August of 2019, resulted in 29 health citations. To provide a comparison, the average number of health citations in Michigan are 13.3. The national average is only 8.2 (Medicare.gov, n.d.). Regardless of how you look at it, this is an abysmal number. Looking at the report, many of these citations are for more minor issues that don’t cause significant harm. However, the sheer number of small mistakes adds up to a fairly large issue that indicates this facility lacks attention to detail. Attention to detail is extremely important in healthcare, as it is the difference between giving a patient 0.5mg of a dangerous medication or 5.0mg instead, a 10-fold increase. little mistakes matter.
Some of the violations include not honoring resident’s rights to request, refuse, or discontinue treatment, such as not correctly following end of life choices such as DNRs. One resident was not given range of motion therapy, resulting in increased pain and medical issues stemming from that neglect. Other residents were given medications that were not needed, and doses were not gradually reduced to remove the patients from these unnecessary medications (Medicare.gov, 2019).
Their fire safety inspection results, at least, are much better than the health inspection results. Where the national average of fire safety citations is 3, and the Michigan average is 4.6, Hallmark Living Holland only had 1 citation in August 2019. Their overall rating is still only 1 star, but at least they take fire safety seriously, even if it is by accident.
Correcting the issues of this facility is a tall order. I would say that their medical staff needs to be retrained on proper procedures in a massive overhaul to improve quality of life for patients. Perhaps sending employees for intense training in small batches would be a good idea, so that it doesn’t stretch staff too thin at the facility while getting these staff members up to speed on what they should be doing to keep residents safe.
